Adoption Agreement

 

The information below is being collected to allow LA adopt to keep a record of placement for those dogs that we advocate for and to transfer ownership and responsibility for this animal.

 

* We will require approval for each protocol prior to adoption

* If any reason the animal will not work out we will take the pet back with no questions asked

If you live in an apartment, townhouse or home owner association complex verify with the management that you're allowed to have a dogs. We will verify with landlord upon approval. Note: many insurance companies are now denying home owners insurance companies to rent to people with specific breeds dogs.

Return Policy. LA adopt has a commitment to all its animals: to care for them for the lifetime of each animal. The Adopter agrees that if he or she is unwilling or unable to care for the pet for its entire lifetime, he or she will contact LA adopt and return the pet to a location specified by LA adopt. however, your adoption donation will not be refunded. The Adopter understands that he or she is responsible for the care of the animal until he or she has a proper foster home. The Adopter agrees that in the event that he or she finds a suitable alternative home for the animal, he or she will contact LA adopt with the name, address and phone number of the proposed new adopter. Any proposed new adopter will be required to complete a LA adopt  adoption application and other requirements for adoption and obtain approval from LA adopt in writing before adopting the pet. Under no circumstances shall the adopter transfer permanent custody, ownership or possession of the dog to any one entity, including family or friends or any entity or agency, without the approval and written consent of LA adopt

 

By signing below, you consent to take over ownership of the dog/cat referenced on this document and release LA Adopt or any of its agents from any and all responsibility for this animal. It is understood that you agree to provide food, water, indoor shelter and medical attention for this animal. This contract transfers ownership of the dog/cat to you and you alone. It does not permit you to transfer ownership to any other party.  If you are at any time, for any reason, unable to continue to care for this animal you will contact LA Adopt immediately, we will reclaim ownership. We don't board our animals, a 30 day notice will be required to properly find a foster home. Your signature on this document implies that you agree to the above and that you will not abandon or surrender this animal to a shelter or other person.
